On January 20, 2026 at approximately 8:30am, Licensing Program Analysts (LPAs) Quimbo and Tse conducted a pre-licensing visit. LPAs met with director of learning success, Michelle "Shelley" Marquez, facility director, Erin Olivas and executive director, Rachel Heck.

Upon LPAs' arrival, children were also arriving to facility. Facility is currently licensed as a combination center with a preschool and toddler component under the existing license number 410500286. This is a single license that is being consolidated with the existing license number.

Applicant requested for 133 children total: 16 infants (6 weeks to 18 months old), 50 toddlers (18 months to 36 months) and 74 preschool children. A fire clearance for 143 children (16 infants, 20 toddlers and 107 preschool children) was obtained December 22, 2025.

At approximately 9:00am, there are 62 children (8 toddlers and 54 preschool age) with 11 teaching staff present. All teaching staff have fingerprint clearance and are associated to the facility.

The center operates Monday through Friday 7:00am to 6:00pm. LPAs toured and measured the day care areas at the facility.

The infant component has one classroom: "Spruce". The toddler component has three classrooms: "Maple", "Willow" and "Mini Willow". The preschool component has three classrooms: "Birch", "Cedar", and "Redwood".
Infant indoor measurements are:
The indoor infant classroom measurements are 560.63 square feet which will accommodate the request for 16 infants.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Toddler indoor measurements are:
The indoor toddler measurements contain a total of 1444.18 square feet which will not accommodate the request for 50 toddlers.

Preschool indoor measurements are:
The indoor preschool measurements contain a total of 2315.47 square feet which will not accommodate the request for 74 preschoolers.

LPAs observed classrooms to be complete with age appropriate furniture, including tables and chairs. Toys and materials for children's use were observed to be in good condition. Classrooms were observed to have rugs. Per director, rugs are vacuumed daily. Storage for children's personal belongings are located both inside and outside the classrooms. LPAs observed children's individual cubbies to be labeled with children's individual names.

LPAs observed classrooms to be equipped with a fire alarm system, smoke detector, carbon monoxide detector, first aid kit and fully charged fire extinguisher. LPAs tested at least one carbon monoxide detector which was observed to be in working condition. Infant classroom include soft toys and padding that was observed to be new.

Children sleep in their designated classrooms. Preschool and toddler children sleep on floor mats. Sheets and blankets are stored in individual cubbies so that they are not touching each other. LPAs observed floor mats to be stored appropriately when not in use. Per director, sheets are sent home weekly for laundering. Napping area for infants is physically separate from the activity area for infants. LPAs observed cribs in napping area to be free of loose articles and objects. Per director, sheets will be provided by infants' families.
LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at:
https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed facility representative of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website
at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Infant program sinks and toilets:
There is one restroom in the infant classroom that is exclusive for infant's use. The infant restroom is equipped with 2 toilets and 2 sinks which will accommodate the request for 16 infants.

Toddler program sinks and toilets:
There are two restrooms for toddler's use. There are 6 toilets and 3 sinks accessible which will not accommodate the request for 50 toddlers.

Preschool program sinks and toilets:
There is one shared restroom between the "Birch" and "Cedar" classroom. There is an additional restroom in the "Redwood" classroom. There are 7 toilets and 8 sinks accessible to preschool children which will accommodate the request for 74 preschool children.

LPAs observed toilets and sinks to be in working condition. There are diaper changing tables in classrooms that are within arm's reach to a sink. Changing tables are equipped with moisture resistant materials and with sides of the changing table that are at least 3" high. Diapers and wipes will be provided by children's families and are stored appropriately, labeled with children's names. LPAs observed garbage bins with a tight fitting lid next to the diaper changing tables.

Infant outdoor measurements are:
The infant outdoor measurements contain a total of 163.36 square feet which will not accommodate the request for 16 infants. The applicant representative will submit an outdoor rotation waiver for the infant component.

Toddler outdoor measurements are:
The toddler outdoor measurements contain a total of 2314.54 square feet which will not accommodate the request for 50 toddlers. The applicant representative will submit an outdoor rotation waiver for the toddler component.

Preschool Outdoor measurements are:
The preschool outdoor measurements contain a total of 11,086.80 square feet which will accommodate the request for 74 preschoolers.

Entire outdoor area is fully enclosed and fenced. The infant, toddler and preschool outdoor areas are physically separated from one another. There are a variety of outdoor toys, materials and equipment that are in good condition. LPAs observed sufficient shade throughout the outdoor areas. There are play structures in the outdoor areas that have resilient padding underneath. The infant outdoor area will have soft padding underneath for crawling infants. LPAs did not observe any pools, spas or bodies of water on site in any outdoor area.

There is a kitchen in the "Willow" toddler classroom. Food service is provided that includes snacks and lunch. Reusable utensils are utilized and washed on site. Drinking water is available with children's individual water bottles that will be labeled with their individual names. Children's individual bottles can be refilled on site through the filtered water pitchers that is also refilled through the kitchen sink.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

Children are signed into and out of program through an electronic application. Each authorized representative has a unique code number that is utilized when children are signed into and out of program. The isolation area for sick children is located in the director's office. LPAs observed office to be equipped with a napping mat and quiet activities. Medication is stored in children's designated classrooms, in a high shelf, inaccessible to children.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan for Providing IMS must be submitted to the Department. Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section
1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Facility representatives were reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Facility representatives were also reminded that infant, toddler and preschool children cannot co-mingle. Facility representatives were reminded an infant's needs and services plan must be updated quarterly.

Based on indoor square footage, maximum capacity facility can be licensed for will be 123 children: 16 infants (6 weeks to 24 months), 41 toddlers (18 months to 36 months) and 66 preschoolers (2 years to 6 years old).



During today's visit, LPAs received an updated Child Care Center Application (LIC200A) with the updated capacity of 123 children.

-Updated facility sketches (LIC999) with correct component labels.
-Submission of outdoor rotation waivers for the infant and the toddler components.
-Centralized Application Bureau (CAB) final approval of application.

Facility number 410500286 will close per CAB's final approval.

A notice of site visit was given to facility representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
