On 3/04/2025, at 11:59 AM, Licensing Program Analyst (LPA) Joaquin Mendez arrived to conduct an unannounced Case Management inspection at the Iriarte Family Childcare Home (FCCH). LPA met with licensee’s sister in-law Mia Denise Dean. Mia Denise Dean informed the LPA that the licensee Patricia Iriarte was not home now (12:05 PM) but was expected to return in the next 15 minutes. LPA observed that there were no children present at 12:06 PM.

LPA called the licensee at 12:10 PM and asked when the licensee would be returning. The licensee stated she was headed back to town and would be home in the next 15-20 minutes. The LPA left the FCCH at 12:10 PM and returned at 12:30 PM.

At 12:30PM the LPA met with the licensee Patricia Iriarte and explained the reason for the visit. LPA observed 3 adults in the home during today’s visit. LPA confirmed via LIS, all adults have CRC (Criminal Record Clearance). Additionally, LPA observed one (1) child in care during today’s inspection. LPA notes the child belongs to the licensee. Licensee was advised that any changes in hours of operation should be notified to the department. The licensee stated she was taking her grandmother to the doctor and specifically chose a time when there would not be any daycare children.

LPA discussed the purpose of the inspection with the licensee. FCCH operates from Monday to Friday, 5:30 AM to 5:30 PM, and Saturday 5:30 Am to 2:00 PM. The licensee is updating care to the children ages to reflect care to children aged 0 to 13 years old. The FCCH is adding an additional area to the daycare which will be the backyard area. LPA received an updated LIC279 and LIC999 during today’s inspection.

The licensee submitted an application on March 04, 2025, to include an additional day care area in the home. The additional space is the backyard area with covered patio. The licensee informed the LPA that per Early Child Development the backyard is required to be in the program. The backyard will be used as a designated day care area for all the children registered to the FCCH.
· The backyard is fully enclosed with wood and brick fencing. LPA observed the footing to consist of brick and astro turf.
· Age-appropriate toys were not observed and licensee states once approved she will facilitate the needs for children.
· LPA observed a large glass table under the patio. The licensee stated the glass table will be removed by the end of business day March 5, 2025. A photo will be sent to LPA Mendez as confirmation by text or email. Business card was provided.
· LPA observed a shed in the backyard which was locked and inaccessible. The licensee opened the shed and it was found to be storage for yard tools and BBQ equipment.
· LPA notes that the west side of the backyard area is inaccessible by means of a metal fence. Inaccessible area is where the three (3) family pets reside. LPA reminds the licensee to check outside prior to allowing children access.
· Additionally, the LPA reminded the licensee the importance maintaining constant supervision of children in care.

Licensee is current with the required training.
· Pediatric CPR and First Aid expires on 6/16/2026.
· Mandated reporter training expires on 1/03/2026.

As of this date, March 4, 2025, the following areas will now be accessible to day care children: Living Room, Dining area, Kitchen, hall bathroom, and fenced Backyard.
The off-limit areas are; three (3) bedrooms, one (1) bathroom, laundry room, and garage.

During the exit interview, the LICENSEE, Patricia Iriarte confirmed that there are no Registered Sex

Offenders living in the facility and LPA completed the RSO profile in FAS on 3/03/2025.

No deficiency was cited during today's inspection and effective as of, 3/6/2025 the Backyard is approved for use as a designated day care area.
