25 | Licensing Program Analyst (LPA), Mel Matos, met with Angel Torno, Director, for an for an unannounced Required - 3 Year inspection. LPA notes that the infant program is licensed in Rooms 1 & 2 on the campus of Fairwood Elementary School. The infant program is licensed to serve infant children, ages 6 weeks to 2 years of age. The Facility also has a preschool program (#430703885) in Rooms 3-6 and a school age program (430709874) in Rooms 7 & 8 on the same site. LPA toured the indoor and outdoor areas of the Facility with Angel during today's inspection. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), Menus, and Activity Schedule.
LPA reviewed five children's and seven staff files (6 teachers & 1 aide) during today's inspection. Each child's file reviewed contains the required documents; including the Information and Emergency Information form (LIC 700) immunization records, Individual Infant Sleeping Plan (LIC 9227), and Needs & Services Plans. Needs & Services Plans also include individual feeding plans for each child. Director states that the Needs & Services Plans are updated at least quarterly or sooner if needed. All staff files reviewed contain the required documents; including transcripts/verification of experience/immunization records, and Health Screening Report. All staff have current certificates of completion of the Mandated Reporter Training for Child Care Workers on file. Last fire/disaster drill was completed on August 26, 2024. Director and three staff has current CPR and First Aid certifications on file. Director understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities (field trips).
LPA observed that the teacher/child ratio was in compliance during today's inspection. LPA observed five infant children with three teachers in Room 1 and 13 infant children with three teachers & one aide in Room 2 during today's inspection. LPA notes that the Facility is Title 5 funded and thus Title 5 ratios apply. Director understands the conditions, limitations, and capacity specifications of the Facility license. Angel understands that infants shall be visually supervised at all times and never left unattended. Any child(ren) who exhibit symptoms of illness including, but not limited to, fever or vomiting, are not accepted in care. Any child(ren) who become ill during the day, shall be isolated in the director’s office area.
LPA observed that Rooms 1 & 2 are clean, safe, and sanitary for children, staff, and visitors. The Facility has adequate ventilation, including central air conditioning/heating. LPA observed all furniture and equipment is in good condition and safe for the children. The infant changing tables have raised sides that are at least 3 inches in height. The infant changing table(s) have a vinyl changing pad that is at least 1 inch think and in good condition. Napping equipment is appropriate (cots in Room 2 and cribs in Room 1). |
