Licensing Program Analyst (LPA) Janette Cruz met with Emma Nie, Assistant Director and Claire Brady, Facility Director, to conduct an unannounced Plan of Correction (POC) inspection. Purpose of today’s inspection: verify completion of the Plans of Correction resulting from the previous case management inspection completed on 04/28/23. LPA toured indoor and outdoor areas of the facility. Census was taken, LPA observed that child to staff ratio were in compliance during today's inspection.

The facility was issued the following Type A deficiency on 04/28/23:
1. Section 101229(a)(1) Care and Supervision - Facility self reported that a preschool child, C1, was left unattended and wandered into a classroom for about a minute on 3/31/23.

LPA notes that plans of correction for Type A deficiencies cited on 4/28/23 were submitted by Shelley Gonzales, facility Regional Manager to LPA Cruz via email on 4/30/23.

LPA observed that the copies of Notice of Site Visit and copy of licensing reports from 4/28/23 was not posted in the facility as per posting requirements of Health and Safety Code 1596.8595. Emma and Claire also stated that the LIC9224 Acknowledgement of Receipt of Licensing Reports have not yet been signed by parents as per AB633 requirements. LPA provided regulations copy of Health and Safety Code 1596.8595 to Emma and Claire during today's inspection.


Exit interview conducted and report was reviewed with Director, Claire Brady. Deficiencies were cited during today's inspection.
