Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Required- 1 Year inspection. LPA met with Assistant Director Marissa Munoz and explained the reason for the inspection. Present during today's inspection were 20 children and at least 8 staff.

There is a board to post required postings, such as license, parents' right's, and car seat law. There is a working phone at the facility. LPA reviewed sign in/sign out sheet. LPA reminded Assistant Director that all children need to be signed in and out.

LPA toured in the inside and outside of the facility with Assistant Director. The classroom that is adjacent to the front desk area had a cabinet with sliding doors. The door has a plastic latch, but was not catching to prevent items inside to be inaccessible to children. LPA observed that there were two bottles of cleaning supplies inside, which is accessible to the children. Staff removed the two bottles of cleaning supplies and placed in the locked cabinet under the changing table. All other disinfectant, cleaning supplies, and other items that are dangerous to children were inaccessible to children. Toys and equipment were observed to be age appropriate and in good condition. There were no baby walkers observed during today's inspection. Changing tables were within arms reach of a sink and had the required sides and padding. Trash can for waste have a tight-fitted lid. The last fire/disaster drill was conducted on 05/16/2022. Assistant Director stated that there are no weapons, such as firearms, stored in at the facility.

The outdoor activity space is fenced. Shaded rest area is provided through building overhang and trees. Area around play structure has resilient material. There were no bodies of water observed during today's inspection.

LPA reviewed sleep log for the two infant rooms. Assistant Director stated that they do not do sleep log for the one years old. Facility started to do sleep logs during today's inspection. LPA observed that sleep logs has the name of the child, the date, time check, staff's initial, and any comments. LPA provided Assistant Director with PIN 20-24-CCP. Facility included the information for the LIC 9227: Individual Infant Sleeping Plan on their Needs and Service Plan. LPA observed that the Needs and Service Plan does not include Section C, where parents can put the date they observed their child roll from front to back and vice versa or Section D, where staff put the date that they observed the roll from front to back and vice versa. Facility stated that they will start to have parents fill out the LIC 9227. A copy of the form was provided during today's inspection. LPA discussed the safe sleep regulations with Assistant Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Assistant Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

There is fridge in the two infant rooms to store food and bottles. LPA observed that all bottles had the child's name and the date on it. Food for the one years old are prepared and provided by the facility. Drinking water is provided through individual water bottles.

Facility does not provide Incidental Medical Services (IMS) at this time. Plan for Providing IMS is on file. Assistant Director stated that she will email copy of facility roster to Licensing by 5PM on 07/22/2022. 10 children's files were reviewed during today's inspection. The records reviewed include but not limited to needs and service plan and immunization records. C-1's needs and service plan was last updated on 01/2022. C-2's Needs and Service Plan was last updated on 02/07/2022. LPA reminded Assistant Director that needs and service plan needs to be updated on quarterly.

11 staff files were reviewed during today's inspection. The records reviewed include but not limited to transcript and mandated reporter training. S-1 and S-2 last completed the Mandated Reporter training on 03/14/2020. S-3 did not have Mandated Reporter training on file. Assistant Director stated that staff are in the middle of completing Mandated Reporter training. LPA also discussed with Assistant Director about the conversation of quarter units to semester units.

All staff present have cleared fingerprints. Assistant Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

As a result of this inspection, three Type B deficiencies and two technical violation were issued. Exit interview conducted and report was reviewed with Assistant Director Marissa Munoz. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
