13 | Licensing Program Analyst (LPA) Martha Jimenez-Villanueva conducted an unannounced inspection to deliver findings for the above allegation. LPA met with Director Marie Nunez explained the purpose of today's visit.
Based on interviews conducted, records obtained, and evidence gathered during the investigation process, it is concluded that although the allegation listed on this complaint may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. The allegation is therefore UNSUBSTANTIATED.
Exit interview was conducted, where this report was reviewed and discussed with Director Marie Nunez. A copy of this report was also provided and appeal rights were given. A notice of site visit was issued and must be posted on or adjacent to the interior side of the main door into the facility for 30 consecutive days. |
