This an amend report:
Licensing Program Analyst (LPA) Martha Jimenez-Villanueva met with Director Marie Nunez and informed her the purpose of this inspection is to deliver findings on the investigation for the allegation listed above. LPA observed seven children with one Teacher and one Aide Teacher (infant room) and four children and one teacher (toddler room) . Staff members were present providing supervision to the children.

This Department has investigated the allegation, based on records review, evidence and interviews during the investigation process, which were conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Not deficiency was issued. The facility was not informed about waxed floors and staff tried to eliminate the odor before children arrive. The intention was to eliminate odor as soon as possible for safety and health to the children in care.

Exit interview was conducted, where this report was reviewed and discussed with Director Marie Nunez. A copy of this report was also provided and appeal rights were given. A notice of site visit was issued and must be posted on or adjacent to the interior side of the main door into the facility for 30 consecutive days.
