On 11/21/2025 at 9:45am, Licensing Program Analyst (LPA) Christina Uribe conducted an unannounced Complaint Investigation Visit for the above allegation of a qualifications violation. LPA met with site director, Saman Tafazoli. Also present at the time of today's visit is 4 staff and 13 day care children.

This agency has investigated the complaint allegation that facility operated without a qualified director. During the course of the investigation, LPA Uribe conducted interviews with involved parties and potential witnesses and reviewed relevant documents. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

Notice of Site Visit was given and must be posted for 30 days. Exit interview conducted and report was reviewed with the site director, Saman Tafazoli.

On 11/21/2025 at 9:45am, Licensing Program Analyst (LPA) Christina Uribe conducted an unannounced Complaint Investigation Visit for the above allegation of a record keeping violation. LPA met with site director, Saman Tafazoli. Also present at the time of today's visit is 4 staff and 13 day care children.

During the course of the investigation, LPA Uribe conducted interviews with involved parties and potential witnesses and reviewed relevant documents.Based on evidence received which was reviewed, the preponderance of evidence standard has been met, therefore the above allegation that facility staff do not have complete required personnel records is SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, Article 06, Section 101217(d) - Personnel Records is being cited as a Type B Violation on the attached LIC 9099D.

Report was reviewed and a notice of site visit was given and must remain posted for 30 days. Appeal rights were given. Exit interview was conducted with site director, Saman Tafazoli.
