On 12/18/2025, Licensing Program Analyst (LPA) Andy Yang conducted an unannounced complaint investigation. LPA met with Director, Hsini Huang and discussed the complaint allegation with them, and to deliver the complaint allegation listed above. LPA toured the indoor and outdoor areas of the facility. Present for this investigation were Director, (8) staff, and (20) children.

LPA conducted interviews with the Director, staff, and children, records reviewed, and evidence gathered. LPA observed cracks on the cement walkway and a gap near the artificial grass in the outdoor play area which can pose a potential hazard, making the playground environment not safe. Based on interview with a Staff (S1), S1 had observed a child (C1) fall into the gap area near the artificial grass substaining an injury. S1 immediately tended to the C1's injury with an ice pack, and the parent was on the same day.

Until repairs or upgrades are completed to the potential hazards of the outdoor play area, Director will barricade access to the area and make it off limits.

Based on LPA’s observation and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED California Code of Regulations, 101238.2(d)(1), and Type B deficiency is issued on attached LIC 9099D.

Notice of site visit issued and must remain posted for 30 days. Appeal rights given.
