32 | Based on the Staff Interview, that on 7/30/2024 at 10 am, during S1 break, S1 saw a mice crawled from underneath the kitchen’s refrigerator towards to the kitchen door. The kitchen is a shared space for the Staff and Children’s utensils and food storage. S1 was seating on a chair adjacent to the kitchen door. S1 attempted to move the next chair, the mice crawled back underneath the facility’s refrigerator. S1 stepped out of the kitchen, closed the kitchen door and reported the incident to the S2. The Licensee was informed via phone and requested pest control. Pest control and Licensee came in at noon. Pest Control had an assessment and set up traps. The kitchen remained off limits that day. Food utensils were brought from a nearby grocery store to use in the facility. Lunch was outsourced from a third-party provider.
At 11:15 am, LPA observed the facility’s kitchen was free of rodent droppings (poop) and the kitchen appeared to be clean. At 11:24 am, LPA observed rodent droppings (poop) in the locked Cleaning Supply Storage (Off Limit Area) where the heating, ventilation, and air conditioning (HVAC) equipment system is located. The locked Cleaning Supply Storage (Off Limit Area) is in the children’s and staff bathroom.
As per the Licensee’s reporting on Friday, August 2, 2024, 6:32 PM via email to LPA Estoesta. The facility had a mouse sighting in the kitchen. A teacher reported seeing a mouse on Wednesday (July 31) in the kitchen. “We” immediately called pest control, who set traps, trapped the mouse, and cleaned. “We” do not keep any unsealed food in the kitchen area. We inspected, cleaned and sanitized the kitchen area. “Our” janitorial services (that come after school every day) cleaned and sanitized the entire kitchen area.
Based on LPA Estoesta’ s Staff Interview and Record Review, the Licensee failed to follow the Section 101212 (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours, therefore, the preponderance of evidence standard has been met, the above allegation are to be SUBSTANTIATED.
LPA Estoesta informed the Licensee that this report dated 8/6/2024 included a Type B Citation which can be posted as there is a potential risk to the health, safety, or personal rights of children in care.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
