32 | All cribs or play yards meet the safety standards. The cribs or play yards do not hinder entrance or exit to and from the space where infants are sleeping. The mattresses in the crib or play yards are firm and covered with a fitted sheet that is appropriate and overlaps the underside of the mattresses. Each infant’s bedding is used for them only and cleaned weekly or before use by another infant. There are no loose articles and objects, bumper guards or objects hanging above or attached to the side of the cribs or play yards. LPA observed documentation of infant 15 minute sleep log was missing from file. LPA reminded that infants up to 24 months shall have documentation be maintained in the infant's file and be available for Department for review shall include the following: Date, Infant's name, and Time of each 15 minute check.
Licensee uses redirection and time out as forms of discipline. LPAs observed Staff (S2) raising their voice to a child stating that they will place the child in time out if the child continues to misbehave. LPA reminded Licensee that children's personal rights should not be violated, including but not limited to, no corporal punishment, children are treated with dignity, receive safe, healthful, and comfortable accommodations, interference with eating, intimidation, or other actions of a punitive nature.
Records: Licensee CPR and First Aid certificate expired on 12/28/24. Licensee states that they take the CPR and First Aid training online. LPA Luz reminded Licensee that CPR and First Aid training is required to be taken in-person. Licensee Mandated Reporter Training for Child Care Providers expired on 2/28/24. Licensee was reminded of Mandated Reporter Training (AB1207) and CPR/First aid certifications needs to be renewed every two years. A copy of the licensee’s immunization is on file. Last fire drill was on 7/18/24. Licensee was reminded to conduct fire disaster drills every six months. Licensee was not able to locate 1 personnel file for review. LPA reminded that all personnel records shall be maintained at the child care home and shall be available to the licensing agency for review. LPAs did not observe required postings and licensee was reminded of documents to be posted in a prominent, publicly accessible area of the facility. Entrance checklist for Family child care home was provided.
LPA, Luz reviewed six (6) children files during today's inspection for the following records: Notification of Parents Rights (LIC995A), Consent for Emergency Medical Treatment (LIC627), Identification and Emergency Information (LIC700), and Immunization records. Each infant under 12 months of age has an Individual Infant Sleeping Plan maintained in the file signed and dated by the infant’s authorized representative. Licensee states that they physically check on sleeping infants. Documentation of infant sleep checks every 15 minutes that notate any signs of distress was missing.
