Licensing Program Analyst (LPA), Oscar Huang, conducted an unannounced continue complaining visit to the Facility today. LPA Huang met with director, Jian(Annie) Deng and explained the nature of today's visit to her.

Based on interviewed with staff, parents, reviews child files, and LPA's own observations for the allegation listed above, LPA learned that staff took children temperatures outside the classroom with the door closed during the drop-off in the early morning that left the children in the classroom unsupervised although on a very short time.

LPA concludes that the preponderance of evidence standard has been met and the allegation listed above is therefore SUBSTANTIATED.

Exit interview conducted with Licensee. A type B deficiency was cited.
NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 DAYS.

Director agreed to submit the Plan of Corrections and to conduct a staff meeting & training to ensure this will not occurred going forward with all staff signatures of attendance to the office prior to the POC due day.

This requirement is not met as evidenced by: Children were left alone unsupervised during the early drop off when staff took children temperature outside the classroom with door closed that the facility did not comply with the section cited above which posed an potential health, safety or personal rights risk to persons in care.

Based on interviewed with staff, parents, and reviews child files, and LPA's own observations for the allegation listed above, it could not be proved or disproved.

LPA therefore concludes that although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.

Exit interview conducted and investigation report was reviewed with director, Jian(Annie) Deng. A notice of site visit was given and must remain posted for 30 days.
