Applicant does not transport, but understands that children cannot be left alone and unattended in parked vehicles.
Applicant has a valid CPR/1st Aid, which expires on 06/01/2024. Applicant completed the Mandated Reporter training on 11/01/2021. Applicant was reminded that the Mandated Reporter training requires renewal every two years. Her immunization records for measles, pertussis, and influenza are on file.
The adults 18 and over living in the home are Applicant, her spouse, and her son. Applicant's minor granddaughter also lives in the home. All adults have cleared criminal record, child abuse index, or exemption. TB test results for adults are on file. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.

Applicant will submit the following:
- LIC 279 with original signature
- Health and Safety certificate
- lease agreement
- proof that nails on the fence were fixed
- proof that the shed was fixed
Exit interview conducted and report was reviewed with the Applicant Ana "Mellie" Dominguez. LPA advised Applicant upon approval of Licensing Management, a license for a Large Family Child Care Home will be granted upon completion of the following:
- proof that the nails on the fenced were fixed
- proof that panels on shed on fixed
- Health and Safety training was completed |
