13 | LPA (Licensing Program Analyst)Anna Morales conducted a Subsequent visit to deliver the findings for the above allegation. LPA met with Director Jian "Annie" Deng.
On 9/20/24 and on 9/25/24, LPA's Anna Morales and Shine Yu conducted interviews with staff and children. Director stated that on 9/18/24, a Parent(P-1) notified her that C-1(Child)came home on 9/18/24 with soiled clothing,and on 9/19/24, Parent(P-1) came to the center to observe C-1's(Child)sheet on the cot, and observed a small urine stain. Staff stated that they did not observe the stain nor observe C-1(Child) leave the center with soiled clothing 9/18/24.
Based on the interviews conducted, the preponderance of evidence standard has been met. Therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 is being cited on the next page. Licensee/Director was informed that failure to correct the deficiencies may result in civil penalties.
Notice of Site Visit was issued and Informed Director to Post in a visible location and post for 30 days.
Exit interview was conducted with Director Jian "Annie" Deng. Appeal Rights were given.
