On 10/30/2025, Licensing Program Analyst (LPA) Shine Yu conducted an unannounced Complaint investigation and delivered the findings. LPA met with center director, Jian Deng (D1).

It was alleged that day care child sustained unexplained injuries and licensee did not ensure child’s records open to inspection by the child's authorized representative.

On 8/8/2025, LPA conducted an initial complaint investigation and reviewed supporting documentation. LPA toured the facility, reviewed children files and interviewed D1. D1 provided children sign in and sign out records to LPA. On 8/8/2025, 10/2/2025 and 10/16/2025, LPA conducted phone interviews with parents. On 10/3/2025, LPA interviewed staff and children during facility investigation.
On 10/30/2025, LPA toured the facilty and observed 8 preschool children with two teachers in room 2. LPA observed two parents signed in their children upon arrival. D1 provided children records for LPA inspection.

Throughout the investigation process, it was found the above allegations are UNSUBSTANTIATED; based on interviews, observations, and information gathered by LPA. A finding that is UNSUBSTANTIATED means although the allegation may have happened or is valid, the preponderance of evidence does not prove it.

Exit interview conducted in Mandarin with center director, Jian Deng. A copy of Appeal Rights was given.
A notice of site visit was issued and must remain posted for 30 days.

On 10/30/2025, Licensing Program Analyst (LPA) Shine Yu conducted an unannounced Complaint investigation and delivered the finding. LPA met with center director, Jian Deng (D1). It was alleged that licensee did not maintain accurate sign in and sign out records for day care children.

On 8/8/2025, LPA conducted an initial complaint investigation and reviewed supporting documentation. LPA toured the facility, reviewed children files, and interviewed D1. D1 provided children sign in and sign out records to LPA. During another visit on 8/5/2025, LPA observed a child being handed to teacher by an authorized representative without signing in. When interviewed, authorized representative confirmed that she did not sign the child in.

Based on LPA’s observations and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. As a result, one type B deficiency was cited on the following page and copy of Appeal Rights was given.
