13 | On 10/30/2025, Licensing Program Analyst (LPA) Shine Yu conducted an unannounced Complaint investigation and delivered the finding. LPA met with center director, Jian Deng (D1). It was alleged that licensee did not maintain accurate sign in and sign out records for day care children.
On 8/8/2025, LPA conducted an initial complaint investigation and reviewed supporting documentation. LPA toured the facility, reviewed children files, and interviewed D1. D1 provided children sign in and sign out records to LPA. During another visit on 8/5/2025, LPA observed a child being handed to teacher by an authorized representative without signing in. When interviewed, authorized representative confirmed that she did not sign the child in.
Based on LPA’s observations and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. As a result, one type B deficiency was cited on the following page and copy of Appeal Rights was given.
Exit interview conducted with center director in Mandarin, Jian Deng. A notice of site visit was issued and must remain posted for 30 days. |
