25 | On 04/06/2026, Licensing Program Analyst (LPA) ,Syeda Bahar conducted an unannounced Case Management – Incident inspection at the facility. Upon arrival, LPA met with Franczesca Francisco, Site Supervisor of Catalyst Kids – Williams. LPA explained to Franczesca the purpose of today’s inspection, which was to follow up on an incident dated 01/20/2026. The incident was reported to the Department on 01/21/2026 by Center Manager Lindsay Masuda.
Present during the inspection were Franczesca Francisco, one staff member, and one child. Later, Center Managers Cindy Lipp, Lindsay Masuda, and Site Supervisor Angela Pena joined the inspection. At 2:20 PM, eighteen children arrived at the facility.
During the inspection, LPA toured the indoor and outdoor areas, interviewed staff, and obtained the facility roster, personnel report, and other supporting documents. It was reported that on 01/20/2026, one staff member had inappropriate contact with a child. As a follow up action, the facility placed the staff member on temporary administrative leave. The parents of the involved child were notified by facility staff.
During staff interviews, it was confirmed that the alleged staff member is no longer working at the facility. Center Manager, Lindsay Masuda stated that the center conducted an internal investigation regarding incident. She also stated that the facility provides staff training on managing challenging behaviors and using positive redirection. Staff are also provided with Health and Safety Guidelines for programs with direct supervision and information on children’s personal rights.
No deficiencies were cited during today’s inspection. Appeal rights were provided. Exit interview was conducted, and the report was reviewed with the Center Managers, Lindsay Masuda and Cindy Lipp. Center Manager, Cindy Lipp signed the report.
A Notice of Site Visit was given and must remain posted for 30 days.
