Licensing Program Analyst (LPA), Marilou Monico, conducted an announced Prelicensing Inspection. LPA met with Applicant Representative, Imad Abdelgawad, Site Director, Maha Dabour, and Office Administrator, Mai Nass. The applicant is applying for a preschool license for 30 children, ages 2 to 5 years old. The center will operate Monday through Friday from 8:00 AM to 3:00 PM. Extended care will be available Monday thru Thursday from 3 PM to 5:30 PM.

LPA toured and measured the indoor and outdoor areas of the facility during today's inspection.

Indoor Activity Space:
ROOM 1: 18.75 x 18.58 = 348.38
14.33 x 7.50 = 107.48
Room 1 Total = 348.38 + 107.48 = 455.86
ROOM 2: 13.58 x 10.42 = 141.50
ROOM 3: 13.58 x 9.00 = 122.22
ROOM 4: 14.33 x 10.92 = 156.48
ROOM 5: 13.75 x 9.00 = 123.75
TOTAL PRESCHOOL INDOOR SPACE = 999.81 sq. ft. divided by 35 sq. ft. = 28 children

Since the total indoor space is measured for 28 children and applicant is applying for a capacity of 30 children, applicant representative submitted an updated Application for Child Care Center License (LIC 200A) during the inspection requesting a capacity of 28 children.

The facility has 2 sinks (30) and 2 toilets (30) available for the children. The children's sinks have hot and cold running water. The hot water was measured today at 105 degrees Fahrenheit. The staff bathroom is located adjacent to the Director's Office. The isolation of sick children will be in the Director's Office and isolation equipment will be provided. Sick children will use the staff bathroom if needed and will be accompanied by a staff member. Staff and children's bathrooms are sanitary and operable. LPA observed appropriate lighting in the classrooms. The facility has central heating and air conditioning.

First aid supplies are located in the hallway cabinet between the staff bathroom and the Director's Office. Cleaning products are stored in the cabinet in the Staff Bathroom inaccessible to children. Site Director stated that there are no weapons or firearms on the premises. LPA observed a fully charged 4A60BC fire extinguisher, functioning smoke and carbon monoxide detectors. Medications will be locked in the hallway cabinet adjacent to the Director's Office.

LPA observed that all furniture and equipment is in good condition and safe for the children. The facility will serve morning and afternoon snacks to the children. There is a kitchen at the facility for food preparation. The kitchen is clean and equipped with refrigerator, dishwasher, microwave, and storage space for food. Plastic utensils and plates will be utilized by the facility. There are 6 tables, 36 chairs, 36 cubbies, and 36 cots for the children. There is adequate equipment, supplies, and materials for the preschool children. Transportation will not be provided by the facility. The center has a working telephone, the phone number is (408) 796-7740.

Outdoor Activity Space:
Outdoor Play Area: 56.92 x 19.33 = 1,100.26
TOTAL PRESCHOOL OUTDOOR SPACE = 1,100.26 sq. ft. divided by 75 sq. ft. = 14 children

LPA observed that the outdoor playground area is surrounded with appropriate fencing and the outdoor surfaces are safe. Shade is provided by trees and sails. There is a climbing structure in the playground with synthetic grass/turf as resilient material.

A waiver request for the playground with staggered outdoor schedule was received during the visit.

There were no bodies of water observed. Site Director was reminded that initial lead testing results shall be received within 180 days from licensure. Following the initial testing, facility must conduct water testing every 5 years after the date of the first test.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Applicant will wait until facility opens to determine IMS needs. Applicant Representative was reminded that all adults 18 and over, responsible for the administration or direct supervision of staff, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA reviewed with Applicant Representative, Site Director, and Office Administrator the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Assembly Bill (AB 633) was discussed. Site Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and resource and referral agencies (R&R) throughout California.

1) Fingerprint clearance for the Site Director.


2) Mandated Reporter Training (AB 1207) and Operations/Record Keeping Orientation certification for Applicant Representative.
