THIS IS AN AMENDED REPORT TO AMEND THE REASON FOR VISIT.

On 02/28/24 Licensing Program Analysts (LPAs) Harsimran Kaur and Janette Cruz met with Applicant Representatives, Sooho Lee and Juhee Do. The purpose of the visit was an announced pre-licensing inspection to change of ownership. Facility already have existing preschool program #434415356 operating in a church site, Calvary United Methodist Church. There are 18 children present during today's inspection. Facility requested total capacity for 36. The child care center will operate Monday–Friday 8AM - 6PM. The fire clearance was approved on December 12, 2023.

LPAs toured and measured the on-limits areas and classrooms located at the facility. The facility has a total of 3 classrooms.

Infant Classroom measurements are:

The Indoor total measurements contain a total of {345.32} square feet, which will accommodate Applicant’s request for 9 infants.

Preschool Classroom measurements are:

The Indoor total measurements contain a total of {952.92} square feet, which will accommodate Applicant’s request for 27 preschool children.

Infant Outdoor Play measurements are:

Outdoor total measurements contain a total of {402.23} square feet, which will not accommodate Applicant’s request for 9 infants.

Preschool Outdoor Play measurements are:

Outdoor total measurements contain a total of {2123.87} square feet, which will accommodate Applicant’s request for 27 preschool children.

Facility has one infant room. LPAs observed a diaper changing table placed not within arms reach to the sink. Facility does not have food preparation area. Per Director, parents will provide food and milk bottles for infants. LPAs advised Applicants that as per Title 22 regulations, Bottles and dishes provided by the authorized representative shall be rinsed and returned to the authorized representative for sanitizing at the end of each day. There are 12 chairs, 2 tables, 1 sink and 1 changing tables, 20 cubbies available for infants.

There are two preschool classrooms at the facility. All preschool classrooms are observed to be clean, safe, sanitary and in good repair. A comfortable temperature for children shall be maintained at 68 degrees at all times. Furniture and equipment were observed to be maintained in good condition, free of sharp, loose or pointed parts. All materials and surfaces accessible to children, including toys, shall be free of toxic substances. All play equipment and materials used by children were observed to be age-appropriate. There are 30 chairs, 4 tables, 2 sinks, 50 cubbies, 2 toilets available for preschool children. The only bathroom used for preschool children is marked as women's bathroom. The men and women's bathroom are used by church members on weekends. First aid supplies, medication, and cleaning products are stored in the upper cabinets in the infant room. LPAs observed containers with tight-fitting lids for the disposal of solid waste and diapers. LPAs observed fully charged fire extinguisher, smoke and carbon monoxide detectors at the facility.


Outdoor area for infants and preschool children were free of hazards including, holes, broken glass dry grasses, and other debris that pose a fire hazard. Outdoor activity space is physically separate from space for child care center components. Any construction or equipment that could pose a hazard in the outdoor activity area shall be made inaccessible to children in care. LPAs advised to fix broken wood panels on fence located at the infant outdoor area. The areas around and under high climbing equipment, swings, slides and other similar equipment was observed to be cushioned with sand and wood chips that can absorb a fall. LPAs observed a concrete surface area located at the infant outdoor space that did not have resilient material to absorb a fall. The playground is enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Shade was provided by trees. Director was advised sandboxes shall be inspected daily and kept free of hazardous foreign materials. LPAs observed no bodies of water.

The Applicants shall take measures to keep the center free of flies, other insects, and rodents. All containers used for storage of solid wastes, including moveable bins, shall have a tight fitting cover that is kept on. LPAs observed no fireplace or open face heaters.

Lunch and snacks are prepared off site. LPAs advised Applicants as per Title 22, if food is prepared off the center premises, the following shall apply: (A) The preparation source shall meet all applicable requirements for commercial food services.(B) The center shall have the equipment and staff necessary to receive and serve the food and to clean up.(C) The center shall maintain the equipment necessary for in-house food preparation, or shall have an alternate source for food preparation and service in the event of an emergency. Commercial foods shall be approved by appropriate federal, state and local authorities. All foods shall be selected, transported, stored, prepared and served so as to be free from contamination and spoilage and shall be fit for human consumption. Drinking water is readily available for the children in the facility via bottled waters.

Storage areas for poisons shall be locked. LPAs advised that Applicant maintains first-aid supplies in a location accessible to staff but inaccessible to children.

Drinking water from a non-contaminating fixture or container shall be readily available both indoors and in the outdoor activity area. LPAs advised per Health and Safety Code 1597.16(a)(1) Lead Testing and PIN-21-21-CCP Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every 5 years after the date of the first test.

Per Written Directive 100700(c)(1) Written Directives for Lead Testing (1) For a license issued on or after July 1, 2022, initial testing results shall be received and posted within 180 days of licensure.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPAs discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPAs also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
LPAs reviewed with the Applicants LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Applicants were informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.


The following corrections are needed before licensure will be considered.
1. Proof of repair on broken wood panels of play yard fence located at Infant play yard.
2. Infant outdoor space with concrete surface to have adequate cushioning material to absorb a fall.
3. Approval of waiver for outdoor space use for infant component.
4. Proof of available sink within arms reach from infant changing table.

An exit interview was conducted with the above items discussed. This report was provided to Sooho Lee and Juhee Do, Applicant Representatives. Final license determination will be made upon submission of complete documents and review by the Licensing Program Manager.
