Licensing Program Analyst (LPA), Marilou Monico, made a follow up complaint inspection and to deliver investigation findings. LPA met with Licensee, Nikitha Kandukuri, and explained to her the purpose of today's visit. LPA toured the facility.

During the course of the investigation, LPA conducted observations, interviewed staff, former staff, parents, and children. LPA reviewed records and obtained copy of children's roster, Personnel Report (LIC 500) with staff contact information, police report, video recording, photos, and other evidence.

Based on interviews and evidence gathered during the investigation process, it is concluded that although the allegation noted on this complaint may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur. The allegation is UNSUBSTANTIATED.

Exit interview conducted and the report was reviewed with Licensee, Nikitha Kandukuri.

A Notice of Site Visit was given and must remain posted for 30 days.
