Licensing Program Analysts (LPAs) Marilou Monico and Elizabeth Berumen met with Applicant Representative/Executive Director, Julie Kennedy, and Assistant Director, Shelly Frost, for the purpose of conducting a pre-licensing inspection in response to a request for change of location and change of capacity. LPAs note that the facility is licensed at 1670 Moorpark Avenue, San Jose, CA 95128 (Facility number: 434408852).

The Preschool Program will operate Monday through Friday from 7:30 AM to 5:30 PM, serving children ages 2 years to 6 years old. The Preschool program will be utilizing Rooms 113 thru 117.

LPAs toured and measured the indoor and outdoor areas of the facility during today's inspection.

Indoor Activity Space:

ROOM 113 (Zebra): 33.333 x 30.083 = 1,002.756 minus 15.795 (encumbered space) = 986.961
ROOM 114 (Elephant): 1/2 (30.333 x 14.667) = 222.447
27.750 x 29.083 = 807.053
Room 114 total = 1.029.50 minus 129.65 (encumbered space) = 899.85
ROOM 115 (Pelican): 29.083 x 30.500 = 887.031 minus 50.243 (encumbered space) = 836.788
ROOM 116 (Bee): 29.167 x 26.000 = 758.342 minus 15.795 (encumbered space) = 742.547

The facility has 11 sinks (165), 10 toilets (150) available for the children. The sinks only have cold running water. There are staff bathrooms labeled as "Ladies" and "Men" located in the Multi-purpose Room in Building B. The sick children will be using the "Ladies" staff bathroom if needed and will be accompanied by a staff member. Sick children will be isolated in the Director's Office/My School Office (Room 105) and isolation equipment will be provided. Staff and children's bathrooms are clean, sanitary, and operable.

There are 13 tables, 95 chairs, 107 cubbies, 92 hooks, and 3 cots for the children. Julie states that Room 113 has exclusive use of the bathrooms located between Rooms 113 and 112 during daycare hours. LPAs observed a latch installed in each of the bathroom's door to prevent anyone from Room 112 to enter the bathrooms. LPAs observed containers with tight-fitting covers for the disposal of solid waste in each room. First aid supplies are stored in the classrooms and in the Director's Office inaccessible to children. Cleaning supplies are stored in the upper cabinet in the Director's Office. LPAs observed smoke/carbon monoxide detectors and adequate lighting in each classroom. The facility has central heating and air conditioning. Medications will be stored in a locked cabinet in the Director's Office.

Julie & Shelly understand that the facility must be kept free of flies, other insects & rodents. Additional furniture, equipment, and supplies will be moved to this location from the Moorpark site once licensed. Drinking water will be readily available for the children indoors via individual water bottles. Transportation will not be provided by the facility.

The facility will be serving morning and afternoon snacks. There is an off-limits kitchen located in the Multi-purpose Room for food preparation. The kitchen is clean and equipped with hot and cold running water, refrigerators, microwave, stove, oven, dishwasher, and storage space for food. Disposable utensils and plates will be utilized by the facility.

OUTDOOR MEASUREMENTS:

LPAs observed that the playground is fully fenced which meets the height requirements. Shades are provided by trees, climbing structure umbrellas, and building overhang. The areas under and around the play structures are cushioned with material (tanbarks) that will absorb a fall. Drinking water will be provided via individual water bottles. No bodies of water were observed.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS was submitted to the Department. Julie and Shelly were reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPAs reviewed with Julie and Shelly the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Fire clearance was granted on January 19, 2023.

Exit interview conducted and report was reviewed with Julie Kennedy, Applicant Representative/Executive Director. LPAs advised Julie that a license for 96 preschool children will be submitted to Licensing Management for the final stage of approval.
