25 | Licensing Program Analyst (LPA), Shine Yu, conducted an unannounced Annual Inspection 12:15 pm and was greeted and met with the Licensee, XiaoLi Bao. Present are licensee, 1 helper and 12 children, including 3 under 2 years old.
LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC610), Parents' Rights (PUB 393) and Earthquake Preparedness Checklist (LIC9148). Facility's operating days and hours are Monday to Friday 8:30 AM to 6:00 PM. LPA observed a current Children's roster. The last fire drill was conducted on 12/05/2025.
LPA reviewed a random selection of 8 children's files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. LPA reviewed the current infant's 15 minutes sleep check log.
LPA also reviewed the staff files for the following records: Employee Rights (LIC9052), Statement Acknowledging Requirement to report Child Abuse (LIC9108), and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and influenza. Licensee's pediatric 1st Aid and CPR certificate expires on 11/16/2026. Licensee's mandated reporter training certificate expires on 11/18/2026.
Licensee accompanied with LPA to tour the facility. The on limits areas inside the home are living room, bedroom 1, 2, dining room and bathroom. LPA observed the fire extinguisher (3A40BC) in the dining room, the smoke and carbon monoxide detector are in living room, hallway and dining room. The bathroom is clean and in working condition. There are age-appropriate toys in the living room and play yard for children. LPA stated the sharp objects need to be inaccessible to the children. The file place is blocked by furniture which is inaccessible to children. Licensee stated there is no firearm in the facility. There is no stairs in the facility.
