25 | On February 20, 2025, at 2:25 PM, Licensing Program Analyst (LPA) Kenneth Madrigal and Mita Partoza conducted an unannounced annual inspection visit, and met with Marie Pendar, Administrator (ADM). Upon arrival, Staff 1 (S1) identified themselves and LPAs were granted access to the facility as LPAs explained the purpose of the visit.
During the entrance of the inspection, LPAs observed 2 residents and 1 staff. LPA toured the facility inside out with ADM which included the living room, kitchen, dining room, two restrooms, two residents’ bedrooms, and four vacant resident bedrooms. The staff area of the facility was also inspected. The front yard and backyard were inspected. Two-day perishable food supplies and seven-day nonperishable food supplies were observed. LPA observed the cleaning product storage area is locked and inaccessible to residents in care. Room temperature was at 68 degrees F, and hot water temperature was measured at 103.1 degrees F in both resident bathrooms.
Based on LPAs observations, food stored in the kitchen freezer has freezer burns, medication storage is not locked which is accessible to residents in care, knives and sharps storage is not locked which is accessible to residents in care, S1 is not associated to the facility, unknown substance stored inside an opened laundry detergent bucket outside in the backyard. ADM stated that there are no chemicals in the bucket. Additionally, an opened soil bag is located outside in the backyard and is easily accessible to residents, floor tiles and two glass windows located outside the backyard is also accessible to residents in care.
Fire extinguisher was last serviced on January 12, 2024. The facility was equipped with smoke and carbon monoxide detectors. In today’s visit, the smoke detectors were not tested by Adm. LPA observed facility first aid kit. LPAs will continue the record review for facility fire/earthquake drill log. The facility's last drill was on January 12, 2024.
