25 | On 01/31/2025 at 9:00am, Licensing Program Analyst (LPA) Jialing "Julianne" Zhu, met with applicant Yissela Alvarez for an announced prelicensing inspection. The applicant lives in the home with one (1) minor child. An Entrance Checklist (LIC 9280) was provided to the applicant. The home was toured to conduct a Health and Safety Inspection. The facility’s intended hours of operation are Monday-Friday from 8:00am to 4:00pm. Applicant intends to serve children ages 5 months – 4 years old.
The home is a single-story home with three (3) bedrooms, three (3) bathrooms, living room, dining room, kitchen, detached garage, front yard, backyard, and patio. The on-limit areas are dining room, Bedroom 2, Bathroom 2, and patio. The off-limit areas are Bedrooms 1 and 3, Bathrooms 1 and 3, living room, kitchen, front yard, backyard, and garage, which will be inaccessible by closed and/or locked doors, barricades, and visual supervision. The isolation area is a section of the dining room. When a child shows signs of illness, he/she will be separated from other children here.
The inside of the home is observed to be clean and orderly, with central heating and ventilation for safety and comfort. LPA observed there are ample safe and age-appropriate toys, play equipment and materials. All toxins, cleaning products, and hazardous materials have been made inaccessible to the children. There is a fully charged 3A40BC fire extinguisher in dining area. LPA observed a working smoke detector and carbon monoxide detector in the dining room. There is a working telephone in the home. There is a fireplace in the dining room, which has been made inaccessible to children. There are no firearms and one small pet in the home.
Play yards are also in the facility with the correct size mattresses and fitted sheets. Applicant stated that bedding will be provided to the children during nap time. Applicant stated that she will be providing snacks and meals for the children. LPA informed applicant that all beddings and food brought from children’s home shall be labeled with the children’s name and stored appropriately. Page 1 of 4. See LIC 809C.
