Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Case Management-Other inspection. LPA met with Director Andrea Marc and explained the reason for the inspection. The purpose of this inspection is to discuss and incident involving S-1 and a violation of personal rights that occurred on 08/15/2023. There were no injuries as result of this incident.

During an inspection on 09/22/2023, Director disclosed that there was an incident involving S- 1. Director stated that S-1 was terminated on 08/15/2023. LPA also reviewed staff file.

As a result of this inspection, a Type A citation was issued. Exit interview conducted and report was reviewed with Director Andrea Marc. A notice of site visit has been issued and must remain posted for 30 days.

LPA Samantha Yip informed Director Andrea Marc that this report dated 11/02/2023 documents one Type A citations which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Samantha informed the Director to provide a copy of this licensing report dated 11/02/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Based on interview and record review, there was an incident on 08/15/2023 involving S-1 and a violation of personal rights. There were no injuries obtained. This poses an immediate health and safety risk to children in care.
