On 10/07/2025, Licensing Program Analyst (LPA), Farida Raja conducted an unannounced complaint visit to deliver investigation findings for the above allegations. LPA met with Director, Jordan Armstrong and explained the purpose of today's visit.

During today's inspection, LPA toured the facility and observed ratios. LPA observed a total of 51 children and 7 staff within the two classrooms toured at the facility. Facility is operating within the ratio requirements during today's inspection.

During the course of this investigation, LPA interviewed director, staff and parents and reviewed relevant records. Based on review of child supervision records from 08/07/2025 to 08/20/2025, LPA observed that sufficient staff were not present for the number of children in the TK/ Kindergarten classroom on several days during this period. Facility did not maintain staff to children ratios per regulatory requirements. LPA reviewed these records with Director during today's inspection.

Based on sign in and out records reviewed from 08/07/2025 to 08/20/2025 and from 10/01/2025 to 10/07/2025, LPA observed several children were missing sign ins or sign outs by staff or authorized representatives. The preponderance of evidence standard has been met for both allegations; therefore, the above allegations are found to be SUBSTANTIATED.

One Type A and One Type B citation was issued as a result of today's inspection. LPA Raja informed Director, Jordan Armstrong that this report dated 10/07/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Raja informed the Director, Jordan Armstrong to provide a copy of this licensing report dated 10/07/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Appeal rights and LIC 9224 were printed and provided to Director.

Exit interview conducted and report was reviewed with Director, Jordan Armstrong.

A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS.

During today's inspection, LPA toured the facility and observed ratios. LPA observed a total of 51 children and 7 staff within the two classrooms toured at the facility. Facility is operating within the ratio requirements during today's inspection.

During the course of this investigation, LPA interviewed director, staff and parents and reviewed relevant records. The complaint alleges that complete paperwork is not obtained from parents/authorized representatives for children at the time of enrollment.

Based on 15 children's files reviewed some children were observed to be missing the Department (Child Care Licensing) address and phone number on the Personal Rights form (LIC 613A) and some were observed to be missing signatures of authorized representatives on some forms, but all children were observed to have all the required forms. Parents' Rights forms, which includes the Department address and phone number were observed to be posted in both classrooms. Based on evidence gathered at this time, it is concluded that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur. The allegation is thus UNSUBSTANTIATED.

No deficiencies were cited as a result of today’s inspection. Exit interview conducted and report was reviewed with Director, Jordan Armstrong. Appeal rights provided.
