32 | Facility representative was interviewed on 6/25/24 at 8:59am and acknowledged that there was a small incident in which a preschool classroom was transitioning from outside play to inside the classroom this occurred between 10:30am on June 4th, 2024. Child (C1) was outside and there was another classroom separate from the preschool classroom that was outside and as the preschool classroom was transitioning to inside, the staff did not do a head count and left (C1) outside. Facility representative denied that C1 was left outside without supervision since there were 2 staff outside standing near the chain-link fence where they always had eyes on C1. Facility representative stated that it was estimated that C1 was outside for 30 seconds.
LPA interviewed staff (S1-S4) on 6/25/24. S1 stated that they work in the toddler classroom and when they were outside with their toddler classroom, they noticed that staff from the preschool classroom were leaving a child behind. S1 stated that they were yelling at staff as they were going into the classroom and staff did not hear them. S1 stated that they told S2, who was with them during outside play time with toddlers, to go get the radio and call the staff from the preschool classroom. S1 stated they told the C1 to stay outside next to them (the fence divided them) and once they had the radio, they called staff from the preschool classroom and informed them that they left a child behind. S1 stated that a staff member came outside to retrieve the child. S2 stated they were outside in the play area with toddlers along with S1. S2 stated they had their kids out in the playground from the toddler classroom and they were playing near the fence and noticed that staff from the preschool classroom were going inside with their kids and saw C1 was left behind and was playing near the fence line of the class toddler room playground. S2 stated that they were yelling at the staff from outside to get their attention and notified (S1) that staff left C1 behind. S1 and S2 banged on the window of the preschool classroom that faces their playground but could not get staff attention. S2 stated that S1 called staff over on the radio to let them know that they had left C1 outside. S1 and S2 both stated they did not take eyes off C1 when they were outside. S2 stated that C1 may have been outside for about 3 minutes or less before another staff from the preschool came to retrieve C1 from the playground. S3 stated that there was no child left unsupervised because there was not an extended period of time and there was only one minute that the child was left outside but they were being supervised by 2 staff from the toddler classroom. S4 stated they work in the preschool classroom and were aware of the incident where C1 was left outside. S4 stated that day during the incident, they had two substitute teachers and S4 was not present during transition from outside to inside because they were inside the classroom assisting two children in the bathroom. |
