On 8/27/24 at approximately 12:10pm, Licensing Program Analyst (LPA) Kayla Danielson and Nicolette Cunningham conducted an inspection to obtain documents regarding an incident that occurred on 8/22/24.
A notice of site visit was given and must remain posted for 30 days.
