On 02/17/26 at 9:48am, Licensing Program Analyst (LPA) Sydney Sims conducted an unannounced complaint inspection and met with Director Niki Bull. It was alleged that Licensee is not ensuring that facility outdoor activity space is free from hazards, specifically that the playground has glass where the children play.

The Director was interviewed on 01/21/26 at 12:50pm and denied the allegation stating that the facility staff sweep the playground before children play on it in the morning time and if any glass is found it is removed from the play ground

Two staff (S1 – S2) were interviewed on 1/21/26 and 2/11/26. S1 had no knowledge of the allegation stating that glass has been found on the playground but that the staff will remove the glass, S1 stated that staff do a sweep of the play ground before the facility opens and after the facility closes. S2 had knowledge of the allegation stating that the children and staff consistently find glass on the playground, S2 stated that staff do not do sweeps of the playground to look for glass.

Four children (C1 – C4) were interviewed on 01/21/26 and had knowledge of the allegation C1 – C4 stated that there is glass on the play ground and that the children will bring the glass to the staff to dispose of.

On 01/21/26 at 2:55pm the facility was toured, and the LPA observed a piece of glass on the playground where the children were playing.

During the investigation LPA conducted interviews with staff and children and toured the play ground that supported the allegation.

Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D.

Exit interview conducted and report was reviewed with the Director Niki Bull. Appeal rights were provided.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
