On April 21, 2023 at 11:30 AM, Licensing Program Analyst (LPA) J. Helton and J. Gifford conducted an unannounced complaint inspection, and met with Director Lisa O'Neal. It was alleged that facility is not being kept free of odors.
The director was interviewed during the visit and stated that on 4/12/23, parents were called and contacted to pick up their children because they were closing the facility for the day.The infant room was closed for the rest of the week. On 4/13/23 they picked up charcoal bags and moth balls (receipt provided) to try and tame the odor. They had 4 people attempt to locate the cause and were unsucessful. Pest control suggested the moth balls. Friday 4/14/23 morning, the smell was better, and they reopened the infant room on Monday. The classroom had windows open and was airing out over the two days and the weekend. There is a fan in the attic and new insulation is being added. Any holes found in the ceiling/roof were covered. Director stated that the mothball smells was pungent, and they removed them.
During the visit 2 staff (S1-S2) were interviewed and both colloborated the dates and stated the smells were giving causing headaches. Continued on LIC9099C

LPAs toured the facility and verified a musty smell, which that is lingering in the hallway entrance to the infant room.

Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D.

Exit interview conducted and report was reviewed with the Director Lisa O'Neal.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
