An inspection was conducted by Licensing Program Analyst (LPA), N. Cunningham in response to an Unusual Incident Report involving injury to a child on 1/30/23. According to the report, Child 1 tripped and hit their head on the sidewalk. The child sustained an injury to their forehead. First aid was applied, and the child's parent was contacted. During the inspection the playground was inspected and photographed. The incident was reported to Community Care Licensing as required. At this time, with the information available, it was determined that no violation of regulations occurred related to the child's injury. This report was reviewed and discussed with the Site Supervisor. All licensing reports are public information and must be made available upon request.

Notice of Site Visit shall be posted for 30 days from today's inspection.

There were no Title 22 deficiencies cited during today's inspection.
