On 02/07/2025 at 08:51am, Licensing Program Analyst (LPA) Kayla Danielson conducted an unannounced complaint inspection and met with facility representative Director Lisa O’Neal. It was alleged that staff threatened daycare child, specifically that a staff threatened to hit a child with a cot.

The Director was interviewed on 11/8/24 at 2:35pm and had no knowledge of the allegation stating that she was not present for the alleged incident. On 11/27/2024 LPA Danielson contacted Director following up on the video footage that was mentioned that possibly captured the incident. Director stated she would be checking with the licensee. On 12/13/2024 while at the facility LPA Danielson asked again about the video footage for the incident and Director stated they asked the Licensee, and the footage is unable to be provided due to the recording only lasting a week before it gets deleted.
Four staff S1-S4 were interviewed on 11/08/24,11/12/2024 and 11/14/2024 and 1 out of 4 staff members confirmed the allegation that S1 threatened daycare child, specifically that they overheard S1 threatened to hit a child with a cot from another room.

Five children C1-C5 were interviewed on 11/08/2024 and 0 out of 5 children had no knowledge of the allegation.

During today’s inspection, the facility was toured 5 staff were supervising 16 children.

Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated.

Exit interview conducted and report was reviewed with the facility representative Director Lisa O’Neal. Appeal rights were provided.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
