On 6/12/2024 at 10:00am, Licensing Program Analyst (LPA) Nicolette Cunningham conducted an unannounced inspection in response to a self reported incident. On 5/2/24, Child 1’s parents informed the licensee that Child 1 took a small fall at home. Later that day, Child 1 fell and hit their elbow in the licensee's living room. The licensee contacted Child 1’s parents and requested they pick up Child 1. Child 1’s parents informed the licensee that they took Child 1 to the doctor and learned Child 1 fractured their arm.

During today’s inspection, LPA toured all on limit areas and photographed the area where Child 1 fell. LPA also reviewed Child 1’s file.

No citations were issued regarding this incident.

Notice of Site Visit shall be posted for 30 days from today’s visit.
