An unannounced case management visit was made to the facility by Licensing Program Analyst (LPA) Y. Yang in response to a self-reported, lack of supervision incident involving child C1 and staff S1 that occurred at the facility on 12/18/24. During today’s visit, the LPA met with the center director, Stephanie Yamato to discuss the incident and obtain additional information.

Director Yamato reported that on 12/18/24 at approximately 10:00am, during a transition from a preschool classroom to the play yard for recess, child C1 was left unattended in their classroom without staff supervision for approximately 25 seconds. Director Yamato stated that C1’s substitute teacher at the time, staff S1, had just stepped into the classroom after relieving C1's primary teacher. Director Yamato stated that S1 believed that she had a total of seven children in her care as they were lining up by the door and waiting to transition outside to the adjacent play yard. Director Yamato stated that while C1’s classmates were lining up, unbeknownst to S1, C1 was in the cubby area of the classroom grabbing their jacket. Director Yamato stated that S1 performed a head count then escorted the seven children from the classroom to the playground and unknowingly left C1 in the classroom. Director Yamato stated that another teacher, S2 walked into the classroom and observed C1 in the classroom attempting to put on their jacket. Director Yamato stated that C1 did not appear to be upset and told S2 that their classmates had already left the classroom. Director Yamato stated that S2 then escorted C1 to be reunited with their classmates. Director Yamato stated there were three other teachers inside a staff break room at the rear of the classroom. However, the three staff members were unaware of C1 being in the classroom at the time.

Director Yamato stated that S2 notified the front office of this incident the same day. Director Yamato stated that she spoke with both S1 and S2 and S1 stated that she did not follow protocol and use the attendance sheet to take roll call before leaving the classroom because she was unable to locate the clipboard with the attendance sheet in the classroom. Director Yamato reported that additional training was provided to S1 regarding using the classroom’s attendance sheet to take roll call before transitioning. Additionally, Director Yamato stated that all staff will be reminded in writing regarding the center’s transition procedures and protocols. Director Yamato stated that C1’s authorized representative was notified of this incident the same day. This incident was also reported to Community Care Licensing as required per regulations.

During this case management visit, the LPA also delivered an amended report to the facility representative for a previous visit made by the Department on 08/23/24. There were no Title 22 deficiencies cited during today’s inspection visit. A notice of site visit was given and must remain posted for 30 days. Technical assistance and best practice recommendations were provided by the LPA during today’s case management visit. This report was read and reviewed with the center director, Stephanie Yamato.
