25 | An unannounced case management visit was made to the facility by Licensing Program Analyst (LPA), C. Castro in response to a self-reported, lack of supervision incident involving child C1 and three staff S1-S3 that occurred at the facility on 01/24/25. During today’s visit, the LPA met with the Center Director(CD), Stephanie Yamato and two Staff to discuss the incident and obtain additional information.
CD Yamato reported that on 01/24/25 at approximately 3:42pm, during a transition from recess in the play yard to the classroom, child C1 was left unattended in the play yard without direct staff supervision for approximately 30-40 seconds. Director Yamato stated that C1’s parent had arrived at the facility to pick up C1 and was present in the play yard having a conversation with S2. C1 had been signed out by S3 who was holding clipboard, and all three staff were aware C1’s parent was present on playground premises. CD Yamato stated that C1 lined up facing the adjacent playground, where the children line up to transition to the extended care program at the end of the day, as C1 on occasion attends the extended care. CD added that at the time the incident occurred there were two facility Staff in the adjacent playground with their classroom having recess, C1 was within line of vision from those facility staff. CD stated that while C1’s classmates lined up by classroom door to walk inside the classroom for their belongings, unbeknownst to staff and C1’s parent, who all walked into classroom, C1 was standing facing the adjacent playground watching the other children play. CD Yamato stated that staff performed a head count, and all children were accounted for since C1 had already been signed out. CD Yamato stated that nor C1 or C1’s parent were upset by the incident.
CD Yamato stated that staff notified the front office of this incident the same day and it was then reported to Community Care Licensing. CD Yamato stated that she spoke with all three staff who did not follow protocol and use the attendance sheet to take roll call during the transition period and assure that parent child contact had been made prior to signing out a child. CD Yamato reported that additional training was provided to take roll call before transitioning and assurance that child and parent have that initial contact before signing a child out.
