25 | An unannounced case management inspection was conducted today 7/19/24 at 1:15pm by Licensing Program Analyst (LPA), Elizabeth Friese. LPA Friese met with Site Supervisor Holly Lewis in response to an Unusual Incident Report received by the Department on 7/18/24 which stated that a child (C1) had injured their foot jumping off of a piece of furniture at the facility.
Holly and 2 staff (S1-S2) were interviewed on 7/19/24 at 1:20pm and stated that staff was providing supervision at the time of the incident which occurred in the kitchen play area of the "Sharks" room. Holly and S1 were not on site when the injury occurred but were familiar with the incident, and reported that C1's parents had taken them to the doctor yesterday, and it was determined that a previous injury from 7/4/24 was probably aggravated by the jumping. S2 was on site and reported that C1 had jumped from the "pantry" component of the play kitchen, approximately 32" high. Pantry has been turned to face the wall. By all accounts C1 is doing fine today. Play equipment is age appropriate and being utilized according to the manufacturer's guidelines.
During today’s inspection, the facility was toured and LPA observed 32 children in care.
There were no deficiencies cited during today’s inspection. A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with Holly Lewis, site supervisor. |
