An annual inspection was made to the facility by Licensing Program Analysts (LPAs), Elpidia Hernandez Torres and Cindy Castro . The facility file was reviewed prior to this inspection. A review of the personnel report on 10/18/23 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. This program is operated by a private entity.

The facility’s operating hours are 06:30AM- 06:00PM, Monday – Friday. The facility was toured inside and outside and the floor and yard plan submitted by the licensee were verified. The items which could pose a danger to children (such as detergents, cleaning compounds and medications) were observed to be inaccessible to children. Licensee reported there are no poisons stored on the premises. The facility was free of flies, insects and rodents. The toys, floors, desks and other equipment and surfaces were clean, toxic free, safe and in good condition. There is uncontaminated drinking water available to children both indoors and outdoors through the use of a water dispenser. The children’s bathrooms were in safe and sanitary condition. A current menu was posted in the front entry way. Food prep areas are clean. Food is properly stored and free of contamination. The playground was free of hazards. The playground equipment and surface areas were in safe condition. There is bark cushioning underneath climbing structures and/or play equipment to absorb falls. There were no bodies of water observed. The licensee stated no weapons are stored on site and none were observed. During today's inspection, staffing ratios were being met and there were 19 children were being supervised by 2 teachers/aides. The facility was operating within the licensed capacity. At least one staff member present during the visit center director possessed current CPR and First Aid certifications. Six children’s records were reviewed at 02:57PM, and contained identification forms with authorized representative information, as well as medical assessments. Two children had medications on file, one was expired, the other was missing a prescription label type B deficiency was issued. Two staff records were reviewed at 02:39PM, and contained health screening forms. The sign in/out procedure was reviewed and in compliance. One staff member S1, did not have criminal record clearance, a civil penalty of $500 was issued and a Type A deficiency.

Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 day, along with the report and Type A citation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Licensee was also informed of the requirement to provide a copy of any licensing report that documents a Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/ guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Based on interview, and record review, Licensee hired employee S1 in May 2023. S1 was not on the facility roster. The licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Licensee sent out S1 to get fingerprinted while LPAs were present. Before LPAs left Licensee was able to show proof of completed LIC 9163 to LPAs. Licensee agreed not to let S1 return until prints come back cleared.
