32 | During the course of the investigation, interviews were conducted with 3 staff (S1-S3) and 5 guardians (G1-G5) between 08/20/24-11/05/24. Staff interviews claimed there are at least 5 to 7 infants present daily, being supervised by a lead teacher and an aide, however, interviews revealed that on at least one occasion staff was left alone with more than 4 infants and on another occasion one unqualified teacher, an aide, was left alone with three infants. There was a separate occasion where two unqualified teachers, aides, were left alone with 10 infants. One guardian (G1) reported there were two occasions when they arrived to drop off their infant, and there was only one staff in the classroom, and the guardian’s infant was the 5th infant present.
LPA reviewed evidence that showed on 08/05/24 there was one staff alone with 5 infants, and on 08/15/24 an aide, who had 0 early childhood education units, was left alone with at least 2 infants in the infant classroom. Facility records and interviews confirmed that S1 and S4, aides (with 0 Early child hood education units) and not qualified to be teachers were left alone to provide care and supervision over infants.
Based on interviews conducted, records reviewed, and observations made, the preponderance of evidence standard has been met and the above allegations are found to be substantiated. The California Code of Regulations, Title 22, Division 12 & Chapter 1, sections 101416.5(b) and 101416.2(b) are being cited on attached LIC 9099D . This report was reviewed with the center director and an exit interview was conducted. Notice of Site Visit shall be posted for 30 days. Appeal Rights were provided.
