25 | Licensing Program Analyst (LPA), Mindy Mohr made an unannounced Plan of Correction (POC) visit and met with Office Administrator Akilah Crossland (S1) to address multiple outstanding deficiencies that were cited on 11/14/2024.
During today's visit, LPA observed 8 infants in care with 2 staff. Interview with S1 revealed there are still three children records that are missing documents. S1 stated she is working with parents to obtain all required documents. S1 requested an extension until 12/30/2024 to complete the children’s files as there are only a few items missing. All other plan of corrections have been met, and the deficiencies cleared.
In addition, LPA Mohr interviewed staff regarding a self- reported unusual incident report that was sent into the Department on 11/26/2024 for an incident that occurred on 11/22/2024.
Exit interview conducted and report was reviewed with facility representative Akilah Crossland. There were no violation(s) of the California Code of Regulations, Title 22 issued during this visit.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
