Licensing Program Analysts (LPAs), Amy Strother and Jessica Gaumann made a subsequent complaint investigation inspection, for the purpose of delivering complaint findings, and met with Licensee Monika Verma (L1). It has been alleged that Staff do not prevent day care children from putting hazardous items in their mouths, specifically that infants have access to small stones and put them in their mouths. It has also been alleged that staff are not providing a comfortable environment for day care children, specifically that the infant room has hard floors without cushioning.

During the initial investigation inspection on 11/05/25, LPAs Strother and Gaumann toured the facility, made observations, and conducted interviews with Licensee (L1) and one staff, teacher (S1). During the investigation, LPA Strother interviewed former staff (D1) and 6 Adults, all parents of currently or formerly enrolled children (A1-A6) between 11/06/25 and 01/21/26.

L1 denied the allegation, stating that there are not hazardous items that can be put in the mouths of children. Staff interviewed shared that the small rocks from the preschool yard were getting into the infant yard, giving infants access to the rocks. Staff stated that they were observing infants put the rocks in their mouths. Staff stated that although they watch the infants closely, try to distract them with items that are safe for their mouths, they did remove the rocks from the infant’s mouths and felt the rocks were a hazard. Staff reported that no children have been observed choking on the rocks.

Parents interviewed, (A1-A6) did not express a concern about the rocks or feel that they were hazardous. One parent stated that she is aware that their child puts everything in their mouth and does the same at home. Another parent shared that they have observed teachers removing items from infants mouths when needed.

L1 stated that she does not have any concerns about the comfort of the building. Staff interviewed shared concerns that there is not enough cushioning in the infant room, specifically stating that there should be more cushioning for infants learning to walk because they fall down a lot or for those that are just learning to sit up. Parents interviewed, (A1-A6) did not express concerns about the comfort of the environment or report any injuries sustained as a result of the lack of cushioning.

Based on interviews conducted and a review of the licensing laws and regulations, there is not a preponderance of evidence that a violation has occurred. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that an alleged violation occurred, therefore the allegations are UNSUBSTANTIATED.

It has been alleged that staff did not ensure the facility was free of pests, specifically dead pests are not removed from the facility in a timely manner, leaving pests accessible to infants and children in care and that rodent dropping have been sighted around the facility.

During the initial investigation inspection on 11/05/25, LPAs Strother and Gaumann toured the facility, made observations, and conducted interviews with Licensee (L1) and one staff, teacher (S1). During the investigation, LPA Strother interviewed former staff (D1) and 6 Adults, all parents of currently or formerly enrolled children (A1-A6) between 11/06/25 and 01/21/26. LPA received and reviewed records on 10/31/25, 11/04/25 and 11/10/25.

During the 11/05/25 interview, L1 stated that she has seen one or two roaches here or there, and they were dead. L1 stated that she has been treating the facility for 3 months, and hired a pest control company, but was unsure of the name. L1 denied ever seeing any evidence of rodents but was told by the former Director that they witnessed rodent droppings around the facility. Staff interviewed reporting seeing roaches in the staff bathroom, the infant nap room, the kitchen, the classrooms and in the office. On 10/31/25 and 11/04/25 LPA received and reviewed photo evidence of what appeared to be dead roaches in the facility. Staff reported seeing rodent droppings by the back door area that leads to the preschool yard. LPA received and reviewed photo evidence of what appeared to by rodent dropping under a folding table in the outdoor area near the back door.

Two parents reported seeing evidence of cockroaches at the facility. One parent interviewed stated that they arrived on a Monday morning at 7:30am to find the facility doors unlocked, windows all open, the lights off and upon entering observed that the furniture had all been pulled away from the walls and dead bugs “everywhere”. This parent stated that they are often the first to arrive and that it was not the first time the door had been unlocked, stating that 85% of the time the door was found unlocked with the lights off, stating that they were told it was an issue with the electronic key pad.

Based on a review of records received by LPA Strother, L1 stated that the facility had pest control done on Sunday, 09/07/25 and the furniture needed to be moved, and the windows left open to air out the rooms. In the records reviewed, L1 states that she could not wait the whole night to let the bugs come out and sanitize afterwards. The remaining parents interviewed stated that they either didn’t spend a lot of time in the facility or were not really looking around and did not observe insects.

During LPA’s visit on 11/05/25 LPA observed a dead cockroach on the floor next to a garbage can, located to the left of the preschool room doorway and took a photo of it. LPA pointed the cockroach out to Licensee, Monika Verma (L1) who did not remove the dead cockroach from the floor. The dead cockroach was left where it was observed. The door remained open while three children were observed to be in the preschool room with one teacher (S1). LPA noted during a review of the records received on 11/04/25 that a dead cockroach was photographed in the same location as LPA had observed on 11/05/25.

Based on LPA’s observations, records reviewed and interviews conducted, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22 is cited on the attached LIC9099-D.

This report was reviewed and discussed with Licensee, Monika Verma. Appeal Rights were provided.
