Licensing Program Analyst (LPA), Amy Strother made a subsequent complaint investigation inspection, for the purpose of delivering complaint findings, and met with Licensee Monika Verma (L1). It has been alleged that Licensee retaliated for staff filing a complaint, specifically that the former Director (D1) was terminated after the licensee learned that she filed complaints on the facility.

During the initial investigation inspection on 11/05/25, LPAs Strother and Gaumann toured the facility, made observations, and conducted interviews with Licensee (L1) and one staff, teacher (S1).

L1 denied the allegation, stating that she fired D1 because of low enrollment and could no longer afford to pay a Director when she herself is a qualified Director and could take on the role. On 11/05/25 S1 stated that she was aware that the former Director (D1) was fired, but did not know the reason D1 was fired.

During a subsequent interview on 01/05/26, S1 stated that L1 gave a lot of reasons why she fired D1, one being that D1 called parents telling them not to bring their children to school on 11/03/25 because there wouldn’t be enough staff and she was mad about that. S1 shared that L1 also said that she was upset about D1 complaining, saying that the heater was broken when it wasn’t.

During the investigation, LPA Strother interviewed former staff (D1) and 6 Adults, all parents of currently or formerly enrolled children (A1-A6) between 11/06/25 and 01/21/26. Interviews conducted with parents A1-A6, corroborated that they had no knowledge of the specific reason former Director D1 was let go from her employment. A2 stated that she had heard from L1 that she was looking to reorganize the school staff. A3 stated that she had an understanding that D1 had concerns about the facility, but did not know why D1 was let go. A4 shared that they believed that L1 and D1 were not seeing eye to eye. A5 stated that there have been a lot of staff changes.

Based on interviews conducted, there is not a preponderance of evidence that a violation has occurred. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that an alleged violation occurred, therefore the allegation is UNSUBSTANTIATED.

This report was reviewed and discussed with Licensee, Monika Verma. Appeal Rights were provided.

Licensing Program Analyst (LPA), Amy Strother made a subsequent complaint investigation inspection, for the purpose of delivering complaint findings, and met with Licensee Monika Verma (L1). It has been alleged that Licensee does not ensure outdoor play space is safe for children, specifically that the licensee is using old style rodent traps to treat the rodent issue.

On 11/04/25 LPA received photos of black boxes said to be rodent bait boxes from a witness. On 11/05/25 LPA observed and photographed the same boxes, in addition to others, located on the preschool play yard.
During the initial investigation inspection on 11/05/25, LPAs Strother and Gaumann toured the facility, made observations, and conducted interviews with Licensee (L1) and one staff, teacher (S1). L1 admitted that she did put rat traps on the preschool yard.

On 11/05/25 LPA's observed several black boxes, located around the preschool play yard labelled “TOMCAT RAT & MOUSE KILLER, Child & Dog Resistant, Disposable Station”. The label states, “This Bait Station is Resistant to Weather and to Tampering by Children and Dogs. For Use Indoors and Outdoors…Do Not Allow Children or Pets to Play with the Bait Station…If Swallowed, call poison control center, doctor or 1-877-332-0755.”

S1 stated during the interview on 11/05/25 that on 11/04/25 when children were playing on the preschool yard, a child picked up one of the rat traps. S1 confirmed that the rat traps were out on the yard during the 11/05/25 Licensing visit, when no children were using the play yard, adding that the traps were also present on the play yard while children were present and playing on the yard on 11/04/25.

During the investigation, LPA Strother interviewed former staff (D1) and 6 Adults, all parents of currently or formerly enrolled children (A1-A6) between 11/06/25 and 01/21/26. Parents A1, A3, A5 and A6 stated that they never observed rodent traps at the facility. Parents A2 and A4 stated that they were aware that there were rodent traps on the preschool play yard. A4 stated that the traps were in areas that was accessible to children but was not aware of any injuries caused by the traps.

Based on record review, LPA’s observations, and interviews conducted, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22 is cited on the attached LIC9099-D.

This report was reviewed and discussed with Licensee, Monika Verma. Appeal Rights were provided.

On 11/06/25 L1 sent a video via text message to LPA Strother of the outdoor play area, demonstrating that all of the black box style rodent traps with poison had been removed from the play yard.

Based on observation, record review and interview, multiple rodent poison bait traps were observed on the preschool playground when the playground was used by children on 11/04/25, which posed a potential health and safety risk to children in care.

On 11/17/25 L1 emailed LPA Strother stating that the bait boxes had been removed from the playground and that pest-control supplies will be stored in an area that is completely inaccessible to children and kept securely locked at all times.
