25 | Licensing Program Analyst (LPA) Robert Maciel made a case management visit for the purpose of following up on a Unusual Incident Report submitted by the facility to the department on 1/16/25 regarding absence of supervision and met with Director Jaclyn Smith (D1).
During today's visit, LPA obtained records, toured the facility, and spoke with staff.
Director stated that on 1/14/25, when children in the Lily classroom were transitioning to the outdoor playground at 3:20 PM, a child (C2) did not want to go outside and proceeded to throw themselves on the ground in protest. As staff Maria (S1) was attending to the child, another child (C1) ran back into the classroom and hid in the bathroom. At 3:25 PM, S2 went back inside to let a child use the restroom at which point she saw C1 in the bathroom hiding under the bathroom sink. S2 called for assistant director Jessica (D2) to explain the incident that had just occurred. D2 spoke to both teachers and reminded them of the school policy of the name to face transitions sheets. D2 also gave them instructions to call either D1 or D2 to assist any outside transitions for the rest of the day. D2 also spoke with C1's mom and made her aware of the incident at which point mom relayed that C2 has the tendency to hide at home and elsewhere.
Since the event, the facility has adapted policy to include C1 being shadowed or placed first in line during transition as well as re-iterating the existing policy of Name to Face checking during transitions.
No deficiencies were observed during today's inspection. A notice of site visit was given and must remain posted for 30 days. Failure to do so will result in a civil penalty of $100. Report was read and reviewed with Director, Jaclyn Smith. |
