Licensing Program Analyst (LPA) Magdaleno arrived unannounced to continue a Complaint Investigation and deliver findings regarding the above allegations and met with Administrator, Morgan Whinery.

Staff are mismanaging resident's medication – Reporting Party (RP) alleges that resident (R1) had not been administered their prescribed medication for “a couple of weeks”. During this investigation LPA made observations, reviewed records, and conducted interviews. Review of R1 Medication Administration Record (MAR) indicated that over the course of thirty-one (31) days, R1 had missed fourteen (14) doses of prescribed medication. Further review of MAR indicated that R1 medication had not been filled after running out. Interviews and review of faxes indicated that facility was sending refill requests to the incorrect agency. Based upon observations, record review, and interviews, there is a preponderance of evidence to prove that the allegations have been SUBSTANTIATED and are valid.

Licensing Program Analyst (LPA) Magdaleno arrived unannounced to continue a Complaint Investigation and deliver findings regarding the above allegations and met with Administrator, Morgan Whinery.

Staff inappropriately increased resident's rent – Reporting Party (RP) alleges that facility increased resident (R1) rent due to a reassessment deeming R1 more dependent. RP claims that R1’s health has not changed, and the rate increase was not appropriately assessed. During this investigation LPA made observations, reviewed records, and conducted interviews. Review of R1 care plan and reassessment did not indicate R1 required higher levels of care. Review of facility rate increase letter presented to all residents indicated that effective July 1, 2025, facility had increased monthly rate for the entirety of the facility and was not a result of increased care needs for R1.We have found that the complaint allegation was UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis.
