25 | A case management visit was made to the facility by Licensing Program Analyst (LPA) Y. Yang to provide technical assistance to the facility. On 06/12/24, Kylie Richardson notified the LPA in writing that effective immediately she is the facility's newly appointed center director. Richardson reported that the facility's previous center director, Alexandria Pepper was stepping down and serving as the facility's associate director. During today's case management visit, the LPA obtained change of director documents from Richardson. Updated facility documents were also obtained from the facility.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the center director, Kylie Richardson. There were no Title 22 deficiencies cited during today's inspection. |
