A complaint investigation visit was conducted today by Licensing Program Analyst (LPA), Sebastian Phouthavong who met with Director, Nadereh Ali for the purpose of delivering complaint investigation findings for the above allegation. LPA conducted an inspection on 02/26/2025 to initiate the investigation and met with Director to discuss the allegations, make observations and request documents. It is alleged that the facility does not provide a safe environment to children in care.

During the course of the investigation, LPA observed a Lysol disinfectants spray bottle located in the daycare children’s restroom and accessible to daycare children. In addition, LPA observed other cleaning supplies located on the kitchen counter with the door opened. The facility has addressed the issue.

Based on the information gathered during this investigation, there is a preponderance of evidence to support the allegation. The allegation is determined to be Substantiated. The following violation of the California Code of Regulations, Title 22; Division 12, was observed: see LIC 9099-D.

Reports citing Type A violations are to be provided to parents/guardians of children currently enrolled by the next business day or the next day the children are in care, and to parents/guardians of children newly enrolled at the facility for the next 12 months from the date of this report.
