25 | A Case Management inspection was conducted at the facility by Licensing Program Analyst (LPA) Y. Yang in response to an Unusual Incident Report (UIR) received by the Department 09/30/2024 involving a child sustaining an injury while in care. The center director, Daysi Lemus reported that on 09/27/24, child C1 was riding a tricycle over wood chips on the center’s main play yard. Lemus reported that when the child’s tricycle became immobilized in the wood chips, the child pulled back on the handlebars, causing the front wheel of the tricycle to lift and the child to fall backwards and hit the back of their head on a concrete pathway. Lemus reported that staff S1, observed the incident and sent the child to the office where an ice pack was applied, and the child’s authorized representative was notified. Lemus reported that the child was transported to be examined by a medical professional and the child’s wound was glued shut.
During today's case management visit, the LPA toured the facility, interviewed staff, reviewed with the center’s lead teacher the facility's child supervision policies, and made best practice recommendations to the lead teacher regarding the physical plant at the facility.
Based on available information, it has been determined that there was no evidence of violations of regulations that occurred that lead to the child's injury. This report was read and reviewed with the center’s lead teacher, Maggie Petersen. There were no Title 22 violations cited during today's case management visit. The Notice of Site Visit shall be posted for 30 days.
