25 | A case management, Plan of Correction (POC) visit was made by Licensing Program Analyst (LPA) Y. Yang today. Previously, a case management visit was made by the LPA on 01/07/2025 and the facility was cited, and a civil penalty was assessed. The facility's acting center director, Autumn Shafer submitted the written POC on 01/20/25 as required. During today's visit, Acting Director Shafer was not present at the facility. The LPA met with assistant director, Maggie Petersen.
During the LPA's visit today, the LPA inspected the facility's playgrounds. Acting Director Shafer notified the LPA on the POC document that construction on the "TK" and "EPS" yards have been completed as of 01/20/25. The LPA inspected the EPS yard and the play structure rated for children ages 2-5 and approved it for use. The LPA inspected the TK yard and observed that there was artificial turf installed under the play structure. The play structure is approximately five feet off the ground at its highest platform. The play structure is now rated for children ages 2-12. The facility representative stated that the previous 5-12 rating was incorrect and an updated sticker was obtained from the manufacturer. Title 22 section 101238.2 (e)(1-2) states that the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls. Sand, wood chips and pea gravel, or rubber mats commercially produced for the purposes above are permitted. The use of cushioning material other than that specified above shall be approved by the Department prior to installation. The facility representative stated that the turf is padded with a hard foam material underneath but was unable to provide additional details about its construction. The LPA advised the facility representative that the TK play yard shall not be used until the alternative cushioning material has been approved by the Department.
Acting Director Shafer also notified the LPA on the written POC document that construction on the "Twos" yard commenced on 01/20/25 with a scheduled completion date of 02/10/25. The LPA reminded the facility representative that no child shall be allowed to utilize this play yard until it has been inspected and approved by the Department. Additionally, the LPA reminded the facility representative that prior to construction or alterations, the licensee shall notify the Department of the proposed change(s). The Department was not notified prior to construction. This report was read and reviewed with the assistant director, Maggie Petersen. There were no Title 22 violations cited during today's case management visit. The Notice of Site Visit shall be posted for 30 days. |
