25 | An unannounced case management visit to the facility was made today by Licensing Program Analyst (LPA) Y. Yang in response to a self reported, alleged personal rights violation incident at the facility on 07/15/24. During today’s visit, the LPA met with the site supervisor, Claudia Avalos to discuss the incident. Interviews were conducted with the facility’s staff members.
Based on the Unusual Incident Report (UIR) received by the Department on 07/19/24, it was reported by the facility that on 07/15/24, staff S2 hit child C1 on the right side of their buttocks. Site Supervisor Avalos stated that though her preliminary investigation shows the allegation to be unsubstantiated, the facility’s internal investigation is still in process. As a result, this incident requires further investigation by the Department. Additional interviews will be completed and a follow-up visit will be made on a later date.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the site supervisor, Claudia Avalos. There were no Title 22 deficiencies cited during today's inspection.
