25 | Licensing Program Analyst (LPA) Amy Strother made an unannounced case management visit to the facility after receiving an Unusual Incident Report (UIR) from the facility on 10/03/25, stating that a parent reported witnessing a teacher pulling a child across the floor by their arms, on 09/25/25. LPA met with facility representative, Site Supervisor, Alma Chavez (D1) to discuss the UIR and received a current roster of children in care.
During today's visit, D1 stated, as written in the UIR, that parent (A1) came to her on 09/25/25 at the time they arrived to pick up their child, stating that they thought they witnessed a staff person (S1) pulling a child (C1) across the floor by their arms when she was signing her child out. D1 stated that she was working outside on the playground at the time of the alleged incident. D1 stated that three staff, S1 – S3 were in the classroom at the time of the alleged incident.
LPA interviewed three staff (S1-S3). Interviews with S1-S3 were consistent with one another, all stating that teacher S1 was letting child C1 know that it was time to go outside during a transition after lunch time. S1 extended their hand to C1, C1 took S1’s hand and then fell to the floor. S1-S3 all stated that there was no struggle witnessed, and nothing concerning about S1’s behavior.
Based on interviews conducted and a review of the UIR, there is no evidence that a personal rights violation occurred on 09/25/25 at this time. There were no Title 22 deficiencies cited during today's inspection.
A notice of site visit was given to licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
