25 | Licensing Program Analysts (Glenn Ouye and Yang Yang) met with the applicant (Dawn Warwick) to conduct a prelicensing visit for the preschool center. The application was received on May 10, 2021. The fire clearance was approved on September 24, 2021.
The facility has five classrooms and a kitchen. There are sufficient furniture, equipment and toys for the children. There is sufficient interior square footage to support 30 preschool children. There are two toilets and one urinal and three sinks for the children. There is a separate staff bathroom. There is one additional room with a bathroom that will be for staff that will live on site. The facility has a first aid kit and all chemicals, medications and cleaning supplies are inaccessible to children. There are functioning smoke and carbon monoxide detectors and a fire extinguisher rate at 2A10BC.
The outdoor activity area is behind the center. The outdoor area is fully fenced. There is sufficient shade for the children. Hydration for the children in the outdoor area is achieved with individual water bottles for the children. There are no bodies of water.
The applicant was reminded that all adults 18 years and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
LPA reviewed with the applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
