An unannounced subsequent complaint investigation inspection was conducted by Licensing Program Analyst Jennifer Velasco (LPA), who met with Director Heather Peters (D1) and Licensee Neeru Vermani (L1). It has been alleged a child (C1) sustained unexplained injury while in care; specifically, C1 developed localized skin rash, possibly diaper rash. It has also been alleged staff left C1 in soiled clothing; specifically, C1's sock and shoe were soiled with fecal matter at pick-up. D1 and L1 denied the allegations and D1 stated no child has sustained injury in care beyond those incurred in the normal course of play and that a rash would have been seen by staff, who would have notified D1, who would have notified the parent(s). D1 also stated no child has been left in soiled clothing while in care at the facility, because staff would have noticed this and cleaned and changed the child into clean clothing during either the scheduled or as needed diaper changes. During today’s inspection, LPA toured the facility, conducted interviews, reviewed facility documents, and observed six classroom staff providing care to 24 children. During the investigation, LPA toured the facility, observed facility staff provide care to children, conducted witness interviews, and requested and reviewed pertinent documents and materials. Witness statements, document reviews, and observation failed to corroborate the allegation that a child sustained unexplained injury in care and failed to corroborate the allegation that a child was left in soiled clothing. Based on interviews and facility documents, the preponderance of evidence standard has not been met; therefore, the above allegations are found to be UNSUBSTANTIATED. This report was reviewed and discussed with D1 and L1. Appeal Rights were provided and exit interview was conducted. All licensing reports are public information and must be made available upon request for at least three years. D1 and L1 were provided with a Notice of Site Visit (NOS) to be posted in the facility for 30 days.
